By submitting this team registration form for participation in the Sika Manu Cup, I acknowledge and agree to the following on behalf of my team, staff, and all associated parties:

1. Assumption of Risk:
   I understand that participation in rugby league involves physical contact and the risk of injury. I acknowledge that Queensland Tonga Rugby League (QTRL), its officials, volunteers, and affiliates are not liable for any injuries sustained by players, staff, or parents/guardians during the event, both on and off the field.

2. Team Responsibility:
   It is the responsibility of our team manager to collect, complete, and submit all required player information prior to the event. This includes accurate and up-to-date contact details, emergency contacts, and any relevant medical information.

3. Medicare Requirement:
   All participating players and team staff must have a valid Medicare card. 

4. Blue Card Requirement:
   All team staff working with minors must hold a valid Blue Card (in accordance with Queensland child safety regulations) and must have submitted this information to the event organisers prior to participation.

5. Code of Conduct:
   All team members, staff, and supporters are expected to behave in a respectful and sportsmanlike manner throughout the event. The QTRL reserves the right to disqualify or remove individuals or teams who fail to comply.

6. Important: Every participant must be registered with a QRL club to take part.
   

The $250 team registration fee is non-refundable, except in cases where we are unable to accept your team into the competition.

Submitting this fee does not guarantee entry. A member of our team will contact you to confirm whether your team has secured a place in the Sika Manu Cup 2025. If we are unable to accept your team, you will be issued a full refund.
